MyADHDReferral.co.uk is an information tool that generates personalised documents to help adults in England navigate the NHS ADHD referral process. It is not a medical service, does not provide medical advice, and does not guarantee any clinical outcome.
The documents generated by this service are not medical advice, a diagnosis, or a clinical assessment. The ASRS-v1.1 Screening Form (Part A) produced is for informational purposes and to support your GP conversation. Only a qualified clinician can diagnose ADHD.
This service is for adults aged 18 and over who are registered with a GP in England. It does not apply to patients in Scotland, Wales, or Northern Ireland, where different NHS referral rules apply.
Documents are generated based on publicly available NHS guidance. NHS policies and provider availability change regularly. We include a date stamp on all documents. You are responsible for verifying that information is current before your GP appointment. We accept no liability for changes to NHS policy after the date of document generation.
Right to Choose is a legal right under the NHS Constitution. However, some Integrated Care Boards operate activity limits that may affect the timing of referrals. We are not responsible for local NHS decisions that affect your ability to exercise this right.
MyADHDReferral Ltd provides this service on an "as is" basis. We are not liable for any clinical outcomes, GP decisions, or consequences arising from use of the generated documents. Our total liability is limited to the amount paid for the service (£19.99).
At checkout you expressly request immediate generation of your documents and acknowledge that, under the Consumer Contracts Regulations 2013, the 14-day cancellation right for digital content ends once generation begins. We offer a full refund within 48 hours if your documents were not generated due to a technical failure on our part. To request a refund, email hello@myadhdreferral.co.uk with your payment confirmation.
Your use of this service is also governed by our Privacy Policy, which explains how we handle your health data under UK GDPR.
These terms are governed by the laws of England and Wales. Any disputes will be subject to the exclusive jurisdiction of the courts of England and Wales.
